School-Based Health Center

The School Based Health Center Program is a joint effort of the Ft. Sam Houston Independent School District and San Antonio Military Medical Center. This program’s goal is to provide medical care for acute illnesses and management of chronic illnesses for DoD-dependents attending Robert G. Cole Middle and High School. The staff includes pediatricians who specialize in the health and welfare of adolescent-aged children. 

WHAT DO SCHOOL BASED HEALTH CENTERS DO? They provided a variety of services, including physical exams, health care services for students that are sick, acute injuries, co-management with a child’s primary care provider of all physical and mental health related issues.

HOW CAN A STUDENT USE THE HEALTH CENTER? This consent form must be signed by their parent or guardian in order to receive health center services. If the student is 18 years or older or emancipated they can sign their own consent form. The consent form is valid for the school year and will be maintained at the school clinic for the availability by the medical provider to ensure the student eligibility for medical services. Parents will be notified by telephone during each clinical encounter to provide adequate feedback regarding their child’s current medical condition. Also parents have the option to be present during the clinical appointment at the high school. Parents or guardians may withdraw consent at any time in writing.

CONFIDENTIALITY: The program adheres to all current laws regarding confidentiality of health services in general and specifically as they relate to services to minor. The Notice of Privacy outlines how we may use and disclose your child’s protected health information at San Antonio Military Medical Center. All student information and documents maintained by the school based health center will be handled by health center personnel only. By operating a health center on school grounds, San Antonio Military Medical Center accepts a unique responsibility to promote a safe and healthy school environment for all students. Completing and signing this form authorizes the Cole School Based Health Center to release and received information as identified in the San Antonio Military Medical Center Privacy Notices.
